Please use this form to apply for membership. A full application form and details will be mailed to the address provided. A sample application form can be viewed HERE and a consent form HERE. 
The 2011 prices MAY be altered so DO NOT send a completed "sample" application form.
The secretary or other club officer must be contact before sending the application forms so that your requirements are met.
